James & Sikes Funeral Home Maddox Chapel Hazel Maddox, of Crestview, Florida died Friday, October 19, 2012 at North Okaloosa Medical Center in Crestview after a long illness. Hazel was born October 15, 1937 in Marianna, Florida. She is the daughter of Claudie and Florence Carpenter.She married Mr. Albert Moran in 1955 in Tallahassee, Florida, they later divorced. She went on to marry Kyle Maddox of Tallahassee, Florida, they later divorced. Hazel moved from Tallahassee, Florida to Crestview, Florida in 2004 to be closer to her daughters. Hazel worked as a waitress most of her working life, retiring in 1990. She was an avid dancer and enjoyed time with her friends and family. Hazel also enjoyed working on crossword puzzles, reading and playing cards. She is survived by her daughters, Jennifer Lynne Billotte and husband Richard of Crestview, Tracy Taylor and husband Brian of Navarre. She has three grandchildren, Desiree Lynne Fiallos and husband Luis of Miami, Amanda Daniels and husband Brian, of Muscatine, Iowa, and Travis Nichols of Muscatine, Iowa. She also has four great-grandchildren and a niece, Irma Sears and husband, Bob of Jacksonville. She was preceded in death by her parents, one brother, Leon Mercer and a sister Agnus Hartridge, two nieces, Sue Harrison and Kathy Slaine. Visitation Services are Sunday, October 21, 2012 at James & Sikes Funeral Home Maddox Chapel, Marianna, Florida at 1pm followed by the funeral service at 2pm. Burial will follow at New Hope Freewill Baptist Church Cemetery in Dellwood, Florida. Donations can be made in her memory to her daughters Jennifer Lynne Billotte and Tracy Taylor.
